Help with completing your NoP form

The notification of practice form needs to be submitted every three years. If you only need to pay the annual retention fee this year, please do not complete the form.

Before completing your notification of practice form, you may find it helpful to read the instruction notes and the Prep handbook.

If the form is not marked correctly, it will be rejected and we will send you another form to complete. This will delay and possibly lapse your registration, which may affect your ability to work or practise.

The most common reason for rejection is a cross ( X ) not being placed in both section B (learning activity) and section C (practice) on the form.

If you do not want to renew your registration, only mark a cross in section A, then move directly to Step 2 and sign and date the declaration. Do not send in your fee if you do not want to renew your registration for another year.

If you need to declare a caution or conviction, mark a cross in section D and complete the reverse of the form. You are also required to declare that you do not have a caution or conviction in section D. Failure to do so will result in your form being rejected and may cause delay in your registration being updated.

To speed up your registration please make sure your form and fee reach us by the 15th of the month you are due to expire.
